How It Works

UnitedHealth Group offers a variety of health and wellness programs and services. Some are available even if you are not enrolled in a UnitedHealth Group medical plan.

Sanvello is an app with on-demand resources to help with stress, anxiety, depression and other conditions – and improve overall mental well-being by helping us develop self-care routines, habits and strategies in our day-to-day lives. Access Sanvello and unlock premium access by downloading the Sanvello app and using your work email address to register. Spouses and domestic partners visit LiveandWorkWell.com (access code: united) to register and learn more.

All employees are eligible regardless of medical plan enrollment.

Rally Stride is a part of the new Rewards for Health program. Get rewarded for taking small steps toward healthy outcomes. Set goals, track progress and get rewarded for improving your physical fitness. Earn up to $50 a month for reaching your daily goals 12 or more days in the month.

Who's Eligible

Employees and their covered spouses or domestic partners who are enrolled in the following medical plans: HSA-eligible plans, Accountable Care Plan, Bind Plan, Doctors Plan, Kelsey-Seybold Primary Care Plan, M Health Fairview Primary Care Plan or WellMed 1st Tier and have completed a health survey and biometric screening. If you are pregnant you do not need to complete a biometric screening to participate in Rally Stride.

How it Works

After completing a health survey and biometric screening, if required, Rally Stride will appear as a recommended action on the Rewards for Health tab. Click on the Rally Stride tile to enroll, then sync your fitness tracking device with Rally. Log into Rally at least once a month to sync your activity and earn rewards for reaching monthly goals.

Zipongo empowers participants to improve eating habits with a host of easy meal planning tools, a robust recipe library, instant grocery ordering and healthy meal delivery. Zipongo makes it easy to plan ahead for a food as medicine everyday approach or to even choose the most nutritious meal options when you are on the go.

All employees, regardless of medical plan enrollment, can visit the Well for support on their weight loss journey. Medical Weight Loss services are part of the UnitedHealth Group medical plan and are offered at select Well clinics to employees and their spouses/domestic partners who are enrolled in an eligible plan, including the HSA-eligible plans, Accountable Care Plan, Bind, Doctors Plan, Kelsey-Seybold Primary Care Plan, M Health Fairview Primary Care Plan or WellMed 1st Tier.

Small changes make big results for people with Type 2 diabetes.

Level2™ is a program offering personalized and data-driven care for individuals with Type 2 diabetes. Through an individualized approach, guided by wearable technologies and personal coaching, Level2 focuses on the reduction of A1C, increasing time in range of blood glucose levels, and the possibility of Type 2 diabetes remission.

Who's Eligible

Employees and eligible spouses or domestic partners who are enrolled in the Bind medical plan AND have a diagnosis of Type 2 diabetes.

How It Works

The Level2 experience includes a small Continuous Glucose Monitoring (CGM) device that is applied to your stomach. It’s easy to use and wirelessly tracks your blood sugar in real-time, directly to an app on your smartphone. When you enroll in Level2, we’ll also give you a Fitbit® activity tracker to help you track daily activity.

Using data from both your CGM and your Fitbit, our experts will provide you with personalized insights. Your team of experts includes registered dietitians, social workers, registered nurses, medical doctors and endocrinologists.

You can talk to or message your personal Level2 care team when it works for you. They’ll be with you every step of the way as you work toward putting your type 2 diabetes in remission by targeting personalized food, activity and medicine interventions.

How It Works

Quit For Life is the nation's leading tobacco cessation program. Quit For Life is a phone-based coaching and web-based learning support service to help quit smoking.

In addition to the Quit For Life program, UnitedHealth Group offers select prescription medications are available at no cost to help stop tobacco use for employees and their eligible dependents who are enrolled in a UnitedHealth Group medical plan. The plan will provide you and your eligible dependents up to two free 90-day courses of smoking cessation medications per year, obtained at a network pharmacy or through OptumRx Home Delivery.

The Fertility Solutions Program connects you with an experienced fertility nurse and provides access to UnitedHealthcare Centers of Excellence providers. As part of your medical plan, infertility coverage, including in vitro fertilization, includes a lifetime coverage maximum of up to $15,000 for medical and $10,000 for pharmacy. In order to receive your medical plan coverage for fertility treatment, you must enroll in the Fertility Solutions Program, are required to meet specific eligibility criteria and receive treatment at a facility within the Center of Excellence network.

Who’s Eligible

Employees enrolled in the HSA-eligible plans, Accountable Care Plan, Doctors Plan, Kelsey-Seybold Primary Care Plan, M Health Fairview Primary Care Plan, Hawaii PPO and WellMed 1st Tier.

To be eligible for the medical plan benefit, you must meet all of the following criteria: be enrolled in an eligible medical plan, be continuously employed for one year, and you or your spouse/domestic partner must meet the medical definition of infertility. You or your spouse or domestic partner must be age 44 or under, if female and using your own eggs/oocytes, or female and age 50 or under, using donor eggs/oocytes. Fertility coverage is for employees and spouses or domestic partners only (dependent children excluded).

How It Works

The Maternity Support Program offers health and educational support from the time you consider starting or expanding your family through the first few months of your baby's life. You gain access to educational materials and a dedicated nurse who can answer questions about a variety of topics, including:

  • Pregnancy
  • High-risk births
  • Newborn caregiving
  • Postpartum self-care

A nurse will stay in touch with you throughout your pregnancy and even after your baby is born. These consultations are designed to address your unique needs to ensure you receive the appropriate care.

The Maternity Support Program is available at no cost as part of your UnitedHealth Group medical plan.

NRS works in conjunction with the Maternity Support Program to identify pregnancies that may be high-risk, and to encourage delivery at a Neonatal Centers of Excellence Network facility. In addition, NRS has experienced Neonatal Intensive Care Unit (NICU) nurses available to answer questions about your baby's care in the NICU and during transition to home. This program gives mothers and babies the opportunity for a better outcome.

Effective Jan. 1, 2021, members enrolled in an eligible medical plan must enroll in the Orthopedic Health Support Program (OHS) to access benefit coverage for certain orthopedic procedures. If you or your covered adult dependent is suffering from back, knee, hip or joint pain, OHS will ensure you receive the best quality, lowest cost care.

Call Health Care Advisor at 800-357-1371 before surgery to participate in the OHS program. After completing the member-identification process, you will be prompted to indicate the reason for your call. At that time, ask for the Orthopedic Health Support program.

A nurse will work with you to ensure you know what is expected of you and you receive the best evidence-based care. If surgery is the right path for you, your OHS nurse can help you locate a Center of Excellence (COE) provider. If you live within 50 miles of a COE, you may be required to use the COE, or the procedure will not be covered.

More than one in three people get some form of cancer during their lifetime, making cancer one of the most prevalent medical conditions. Through the Cancer Resource Center, employees have access to support for any kind of cancer diagnosis, including uncommon, complex or hard-to-treat cancer.

How It Works

Access to the Cancer Resource Services (CRS) Centers of Excellence Network gives patients care that is planned, coordinated and provided by a team of experts who specialize in their specific cancer. Potential benefits include accurate diagnosis, appropriate therapy (neither too little nor too much), higher survival rates and decreased costs. Network benefits are available for patients who receive care at a designated CRS Network facility.

The Cancer Support Program offers intensive patient advocacy, cancer-related education, information and ongoing support during treatment and recovery from cancer. Case-management services are provided by experienced cancer nurses. Patients with any type of cancer can take advantage of this program.

Kidney diseases can be complicated to manage, but with the Kidney Resource Center, employees or dependents with kidney diseases have the support they need to manage their conditions and connect to the right experts and resources.

Kidney Resource Services (KRS) provides access to top-performing dialysis centers and nurse consulting services to support the management of kidney diseases. Kidney transplantation candidates have access to the Transplant Centers of Excellence Network and Transplant Resource Services nurse consulting services.
